Instructions
One short form, once, with a parent or guardian. Five minutes.
- Sit next to a parent or guardian. You both sign.
- Open the form: the CTE acknowledgment form.
- Sign in with your CCHS email (@student.ccusd.org).
- When it asks which course, pick Yearbook Design I or Yearbook Design II. Not sure which? It’s on your schedule.
- It confirms you both read this site and the CRED rubric, and that you understand the two rules with automatic penalties (posting a full name, posting without approval).
- Submit.
- Check your work: you should see a confirmation screen. No confirmation, no form.
What it should look like
Done is a confirmation screen and a parent who knows what you signed up for.
- Signed in with your school email, course picked: Yearbook Design I or II.
- You read this site and the CRED rubric.
- Your parent or guardian read them too, and signed next to you.
- Submitted. Confirmation screen seen. Now the tests.

What the form unlocks, and what it doesn’t.
Signed with a parent or guardian. Once.
Real, in class, 90%.
Real, in class, 90%.
All three done, and your 20 CRED points can be entered.
So nobody is surprised by a rule in February.
Those are different papers. This one is about the class rules.
Lesson
Why a parent signs too
Two rules on this site come with automatic grade penalties: posting a student’s full name, and posting without approval. Those are the kind of rules a parent should hear about from you in August, not from a grade in November. The form is how we know you both did.
What CRED is
CRED is 20% of your grade, graded monthly, same rubric in every CTE class here: taking part, communicating, being on time, taking care of the room and the gear, being someone people can work with. The full rubric. It can’t go below zero, so one bad month can’t sink the rest.
- Your 20 CRED points can be entered
- The two tests count once you pass them
- Your parent knows the two automatic penalties
- Your pages graded any differently
- Extra credit
- Optional
Example
What the course question looks like, and the one mistake people make on it.
You’re in Yearbook I, you picked Yearbook I. The form lands in the right gradebook.
Same form, wrong class. It counts for a class you’re not in. Sign it again with the right one.
Yearbook I and II sit in the same room with the same work, but they’re separate gradebooks. The form has to land in yours.
How to turn it in
It’s a Google Form. Sign in with your CCHS email, pick Yearbook Design I or II, and sit next to a parent or guardian.
Sign the form ↗ Before anything else. The first week of school, or your first week in the class.Rubric
